Admissions Process
The Admissions staff is available during regular business hours to help facilitate a resident's transfer to Crest Haven. Our friendly and knowledgeable Admissions team is pleased to assist in completing all forms necessary to secure a room at Crest Haven, and will advise the patient of eligibility in receiving any community-based resources.

Crest Haven Nursing and Rehabilitation Center accepts Medicare, Medicaid, and many insurance plans, as well as privately paid funds.

The Admissions Director understands the stress that care givers often encounter and are happy to discuss your specific needs and concerns and guide you through the financial process.
